For an MCI it would be best to ask the question on the Bus Conversion Magazine Bulletin Board site. Those guys do coaches, and you'll get more info there then you'll ever want, and then some.

My guess would be that the tensioner itself is the problem. When the bearing/race of the tensioner starts to go any play will throw the belt off.
