Automatic to stick change over??

Curious to see if anyone has done a switch from an automatic transmission to a standard. What are the difficulties you ran into and what else did you have to change? What kind of differences you had in performance and fuel efficiency? Was it really worth it? I'm considering this if I purchase a front engine bus. Also wondering if anyone has done the 6th speed upgrade on the Allison 3060 and tracked the fuel efficiency difference.
