Nurse here. And wife is a doc.
Find a pain clinic or pain doctor that trust you, you do this by being honest, following his plan of care, and doing everything he tells you. The doc can see you via telehealth, order labs, and prescribe. But with if the doc is out of state and you are trying to get your prescription filled in another pharmacy that might be a huge red flag.
Alternatively, find a "loose" primary provider that would just prescribe what ever you tell him. But know that he would probably be a crappy doctor, but you just want pain meds.
You can also just come back every 3 months to get labs and meds.
