Steering dampeners are band aids that only cover up the symptoms, they do NOT correct the problem in the least. Heck, most times they do a pretty terrible job of just masking the symptoms even. You need to figure out what is causing the problem (my guess is on a tire issue). If you choose to replace the dampener just bring it to the parts store with you. Those things are pretty generic and that's probably going to be the easiest way for them to match it up for you.

Do you have bias ply or radial tires and what style of wheels do you have? Daytons (clamped with wedges) or Budds (lugnut like a car hold them on)?

Bias ply tires develop flat spots from sitting which will cause a pretty terrible vibration. Once they warm up and make a few thousand rotations they smooth out. Could this simple explanation be your problem?

start with the tierod and draglink ends, serv ice/adjust the wheel bearings, check and adust the play in the steering gear at high center, then look at the toe in, if none of this finds the problem you need to have the alignment checked it's probably a caster problem.
