hard or easy.. changing out rear end?

hey there I'm thinking I'm gonna change my rear end in my 1994 international Thomas vista and im wondering about how hard is it to replace one? I currently have a ratio of 5.38 and I can't really get my bus over 50-55mph without shooting up the 2700-2800 rpms.. and with my transmission (at545) that is not good for long periods of time (especially in mountains)
